Player Injury and Insurance Claims
All registered players are covered by the FFA National Insurance Program for injuries obtained during official matches and training. However, it is also recommended that players also hold private health insurance as the FFA insurance package only provides limited coverage. If you need to make a claim for an injury incurred, there are three steps required, the first being to complete the online claim submission asap. Please note that you must complete the online claims submission within 30 days of obtaining your injury. Once this initial claim submission has been acknowledged by the insurer you will then be required to submit a more detailed claim form, which is available here.
How to make a Claim
If you sustain an injury whilst training or playing for South Hobart Football Club that requires extensive treatment and you wish to make a claim, you will need to do the following within 30 days of sustaining the injury.
1. Complete and submit a simple online claim form on the Gow-Gates Sport website to alert them that a claim is in the pipeline. This must be completed within 30 days of the injury.
2. Complete the detailed claim form, including the medical report, witness statement, loss of income claim (if relevant) and Club Secretary or Treasurer signature.
3. Lodge the fully completed claim form.
4. Lodge additional documentation as required.
